Appointments
Our physicians operate on an appointment basis. We request that you call
in advance so we can reserve a time for you. Same day appointments usually
can be accommodated. The appointment system helps us ensure that the physician
has ample time for a thorough examination and consultation with the patient.
To ensure you are helped as quickly as possibly when you arrive, please
complete the * Medical History
Form,
* Patient Information Form,
and * Payment Policy. When
completed, please fax them to brunswick office Fax: 912.262.0394 or the
Kingsland office: Fax: 912.729.4654. If you cannot fax the information
to us, please bring the documents with you.
Emergencies
If you experience a medical emergency or a medical situation which causes
you serious concern, go immediately to the nearest hospital. Please call
the office and the doctor will get in touch either directly or through
his staff with instructions.
Workers Compensation
At Summit, work related injuries are a big part of our practice. We strive
to accommodate same day appointments for the acutely injured. Hand written
updates are faxed to the appropriate contact person after each office
visit. We also have a nurse designated to assist patients, employers,
and case managers with any needs they may have.
Prescriptions
Requests for medication refills are taken during regular office hours.
After-hours requests will be considered the following business day when
the medical record can be made available to assure patient safety.
Office Hours
Office hours are Monday-Friday from 8:30am to 5:00pm. After normal office
hours, an answering service will forward all messages to your doctor or
the nurse on call. Your call will be returned as soon as possible.
Fees & Payments
We make every effort to keep down the cost of your medical care without
sacrificing quality. It is requested you pay the co-payment or 20% upon
the completion of each visit. Please ask for the Patient Account Representative
if special circumstances make this a hardship.
Your insurance coverage plays a vital role in helping meet the costs of
quality medical care. Our office will assist you by processing claims
for most insurance groups. Remember, you are responsible for any deductible,
co-insurance or other balance not covered by your insurance company. Questions
about insurance matters are common, and our Insurance Department is available
to assist you.
